Intensive Case Management Access Coordination

What is IntĀc?

IntĀc, Intensive Case Management Access Coordination, is a centralized process and single point of access to Mental Health Intensive Case Management Services for residents of Hamilton aged 16 years or older who are living with a serious and persistent mental illness or concurrent disorder.

The three agencies that provide Mental Health Intensive Case Management Services in Hamilton are:

• Canadian Mental Health Association (CMHA), Hamilton Branch
• Community Mental Health Promotion Program (CMHPP) – City of Hamilton
• Hamilton Program for Schizophrenia (HPS)

IntĀc does not provide counselling services or crisis support.  If you require crisis support, please contact COAST Hamilton at 905.972.8338.

In order to be eligible for IntĀc, the person referred needs to be:

• Living in Hamilton
• Diagnosed with a serious and persistent mental illness or concurrent disorder
• Under the care of a physician (some services require a Hamilton physician)
• Willing and able to participate and to work on wellness goals

Who can refer to IntĀc? Anyone! There is no wrong door.

• Health care professionals
• Community organizations
• Social service agencies
• Families/friends
• Clients self-refer

Steps in the IntĀc Process

• Client self-refers or professional refers by filling out the Service Request Form (see link below).
• IntĀc Coordinator contacts the client and then meets with them to obtain consents and gather further information.
• IntĀc Admissions Team meets to discuss eligibility, which agency may best meet the client’s needs, and/or other resources to recommend. 
• Client and referral source (if applicable) are notified of the outcome.
