Ontario Health Teams (OHT): New Ministry of Health and Long-Term Care (MOHLTC) Plan to Better Connect the Health Care System

Earlier this year, the MOHLTC announced plans to make changes to the current health care system.  The Canadian Mental Health Association, Hamilton Branch has been involved in discussions specific to potential systems changes to improve care for individuals experiencing mental health and substance use issues.  We joined with key community and hospital partners, and together on May 15, 2019 we submitted an application to the MOHLTC to become an Ontario Health Team for Hamilton.  For more information on this process, please click here to view a letter from the Steering Committee to our stakeholders.  A survey link is provided within the letter to receive ongoing updates on this process.
