On July 18, 2019, the Hamilton Health Team was invited by the Ministry of Health to move forward immediately in the process and submit a full application to create an Ontario Health Team (OHT). The team’s initial submission was selected amongst a group of over 150 other applicants from communities across Ontario.
The application and selection process is being led by the Ministry of Health (MOH). While still early in the process, CMHA Hamilton, along with other key community and hospital partners, looks forward to the opportunity to be a successful early adopter of this pivotal new model for integrated health service delivery. We also look forward to learning from OHTs across the province. Most importantly, we look forward to building a system that will improve care, better deliver services and strengthen community supports.
As you can imagine there is a great deal of coordination and work ahead in the development of the HHT application. The Oversight and Coordination Secretariat (OCS) will be meeting to review the Ministry’s guidance documents to inform the development of the full HHT application. We are confident that the planning and coordination work that this team has already undertaken between the self-assessment submission and now will enable us to hit the ground running with this next phase of work.
